Program Event Participant Release Waiver of liability


This Release and Waiver of Liability (the "Release") is executed on this day by the signee below in favor of Love Serving Autism: hereafter referred to as "LSA"; -RALLY After School Tennis Program. The signee desires to work as a Volunteer for LSA, and engage in the activities related to being a volunteer participant in this event. The Volunteer/Participant does hereby freely, voluntarily, and without duress execute this Release under the following terms:

  1. Waiver and Release. Volunteer/Participant does hereby release and forever discharge and hold harmless LSA, and ils successors and assigns from any and all liability, claims and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer's/Participant's work for LSA. Volunteer/Participant understands that this Release discharges LSA from any liability or claim that the Volunteer/ Participant may have against LSA with respect lo any bodily injury, personal injury, illness, death, property damage, or any other liability that may result from Volunteer's/ Participant's work for LSA, whether caused by the negligence of LSA or its officers, directors, employees, participants, volunteers, spectators, agents or otherwise. Volunteer/Participant also understands that LSA does not assume any responsibility for, or obligation to provide, financial assistance or other assistance, including, but not limited to, medical, health, Workers' Compensation or disability insurance in the event of injury or illness.
  2. Medical Treatment. Volunteer/Participant does hereby release and forever discharge LSA from any claim or liability whatsoever that arises or may hereafter arise on account of any first aid. treatment, or service rendered in oonnection with the Volunteer's/Participant's work for LSA.

  1. Assumption of the Risk, The Volunteer/Participant understands that the work for LSA may include activities that may be hazardous to the Volunteer/ Participant, including, but not limited to, vehicle, trailer, equipment, accidents, weather (Acts of God) security duties, contact with clients/public, contact with other volunteers, and contact with the public generally. In connection thereto, Volunteer/ Participant recognizes and understands that participation with LSA events may, in some situations, involve inherently dangerous activities. Volunteer/Participant hereby expressly and specifically assumes the risk of injury or harm in these activities and releases LSA, from all liability for injury, illness, death, or property damage resulting from the activities of the Volunteer's/ Participant's work.
  1. Insurance. The Volunteer/Participant understands that, except as otherwise agreed to by LSA in writing signed by the CEO, LSAdoes not cany or maintain health, medical or disability insurance coverage for any Volunteer/ Each Volunteer/Participant is expected and encouraged to obtain her or his own medical or health insurance coverage.
  1. Photographic Release. Volunteer/ Participant does hereby grant and convey unto LSA, LSA's sponsors and their agents all right, title, and interest in any and all photographic images and video or audio recordings made during the Volunleer's/ Participant's work for LSA including, but not limited to, any royalties, proceeds or other benefits derived from such photographs or recordings. Volunteer/ Participant grants permission to record, edit, lranscribe, duplicate, modify, distribule, and/or publicly exhibit the Volunteer's/ Participant's presentalion or appearance, and use of Volunteer's/Participant's name, likeness. voice, and biographical information in any and all media now existing or hereafter developed, throughout the world, in perpetuity, without restrictions or limitation.


  2. Other. Volunteer/ Participant expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Florida which governs LSA and that this Release shall be governed by and interpreted in accordance with the laws of the State of California. Volunteer agrees that in the event that any clause or provision of the Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

Please note: In connection with your application as a volunteer, please be advised that Love Serving Autism, Inc. intends to use a consumer and/or investigative consumer report as part of your volunteer application and continued evaluation during the course of your volunteer time, should you be accepted as a volunteer.
